All medical records requests, FMLA forms, and disability paperwork are processed exclusively by ShareCare, our authorized third-party records management partner. Use this portal if you are the patient or a legal guardian with legal paperwork requesting a copy of medical records. You will need a valid email address and government-issued ID.

Submit your FMLA or disability paperwork directly to ShareCare for completion. Once submitted, you will be contacted within 24–48 business hours for payment. Forms are completed and delivered within 5–7 business days of payment.

Records are typically ready within 3–5 business days from receipt of your electronic request.

Once submitted, ShareCare will contact you within 24–48 business hours regarding payment. After payment is received, the form is completed and delivered within 5–7 business days.
How do I check the status of my request?

How do I pay for my records or radiology images?
Pay online through ShareCare’s secure payment portal at payonline.hds.sharecare.com.
Can I call University Urology to get my records faster?
No. Our office does not process medical records, FMLA, or disability requests. All requests must go through ShareCare. Calling our office will not expedite your request. Please contact ShareCare directly using the information below.
